Members of Community Care Health Plan have their medical care coordinated through their Primary Care Provider who is a Participating Provider in the network maintained by the health plan. Medical services, including those with referrals by your PCP to a Specialist or other healthcare provider, may require prior authorization or a medical necessity determination to be processed as a covered benefit. Emergency services and urgent care services for emergent medical conditions are available without a referral.
