Community Care Health offers plan options that allow you to tailor your benefit needs to your business needs, choosing what you feel is most important in your health plan. Our plans include Vineyard, Orchard and Harvest HMOs as well as the Summit HMO HDHP and the Glacier HMO/HSA COMP. Additionally, Community Care Health has several other plans to choose from and can customize a plan to work best for your needs. Detailed information about each is available in the Large Group Brochure. For your convenience, Community Care Health provides multiple lines of coverage in one place. Community Care Health Large Groups are offered medical, dental, and vision – or medical with either dental or vision. When bundling more than one line of coverage, discounts may apply.
