Community Care Health provides a variety of HMO and EPO plans, in addition to Dental, Vision and Pharmacy coverage. Because we work closely with your employer to tailor a plan that works specifically to meet your needs, we do not take a one-size-fits-all approach.
For plan details, including breadth of coverage, co-pays, deductibles, premiums, etc., we encourage all members to speak with their Human Resources Department or whomever is in charge of benefits administration. The Community Care Health Member Portal provides 24/7 access to coverage and benefit details, lets you review claims, change your primary care physician, print ID cards and more.
Pharmacy Coverage
Community Care Health has partnered with MedImpact to manager your prescription drug benefit, providing essential information about your prescriptions, lower-cost drug options, and more.
Dental Coverage
Use the Delta Dental portal to learn more about your coverage, how to find the dentist who’s right for you, ID cards and more.
Vision Coverage
Register and login to the VSP Vision portal to understand your vision coverage, how to find the provider who’s right for you, ID cards and more.
Mental Health Coverage
You don’t need a referral from your primary care doctor to get access to mental health services. However, only services offered through plan providers will be covered.
Continuity of Care
New to Community Care Health but already being treated by a doctor outside of our network? You may qualify for Continuity of Care Benefits, enabling you to finish out treatment or have a few more visits before fully transitioning to a Community Care Health in-network provider.

Participation in Community Care Health Public Policy Committee
