Community Care Health (CCH) Claims Processing, Customer Service, and Utilization Management Transition

Dear Provider: 

We are writing to share an important update on our services.  Beginning January 1, 2025, Community Care Health (CCH) will transition claims processing, customer service, and utilization management from third party providers to being fully managed in-house. 

The change reflects our commitment to delivering the highest quality service and ensures a seamless experience for our members and providers. By bringing services in-house we can: 

  • Enhance Quality –  more direct oversight and control to ensure the highest standards  
  • Streamline communication – You will work directly with our team, simplifying support and coordination  
  • Innovate faster – our team will be better positioned to implement improvements and respond to your needs promptly  

Thank you for your continued trust and partnership.  We now want to bring you up to date on how these upcoming operational changes impact you. 

  • New Member Identification Number Format: T000000000 
  • New CCH Member ID Card: A new sample ID card will be provided in an upcoming communication. 
  • New Payer ID for electronic submission of medical claims for dates of service as of 1/1/2025: CCH25 
  • New Mailing address for submission of paper medical claims for dates of service as of 1/1/2025: PO Box 45016 Fresno, CA  93718 
  • New Fax Number for submitting Prior Authorizations as of 1/1/2025: 559-724-4750 and 559-724-4751 
  • New Customer Service number for Members and Providers as of 1/1/2025: 559-724-4995 
  • A new toll-free number will be provided in an upcoming communication. 

For claims with dates of service prior to 1/1/2025, please continue to submit the claim to Payor ID 85729 or PO Box 45026, Fresno, CA 93718.  HealthComp will continue to provide claims services for any dates of service in 2024.

The Provider page offers information on Provider Resources, Pharmacy Coverage for CCH Members, Timely Access to Care among other key materials.    

We are also pleased to announce CCH will be implementing a new Provider Portal effective 1/1/2025.  We will be sending out an additional communication before the end of the year on how you will be able to access the new and improved provider portal.
