New Community Care Health ID Cards and Provider Portal

Dear Provider:

As Community Care Health (CCH) finalizes our transition to all services being in-house, we are providing an update on two items noted in our previous communication dated December 11th regarding the new CCH ID Card and the new Provider Portal.

The CCH ID card has been redesigned as we have modified the CCH ID format and our Payor Information has changed. Attached are sample ID cards for both the HMO and EPO.

We would also like to introduce the new CCH portal. You can access the portal through the CCH website at communitycarehealth.org or through the link provided below:

We have attached instructions on how to register for the new Provider Portal. If you encounter any issues with the registration process, please contact CCH Customer Service at 1-559-724-4995.

Please note that eligibility verifications through Electronic Medical Records (EMR) systems, for example My Chart, will not be available until February 1st.

As a reminder, for claims with dates of service prior to 1/1/2025, please continue to submit the claim to Payor ID 85729 or PO Box 45026, Fresno, CA 93718. HealthComp will continue to provide claims services for any dates of service in 2024. If you have questions regarding a 2024 date of service claim, please call 1-855-343-2247.

For claims with dates of service after 1/1/2025, please submit claims to Payor ID CCH25 or PO Box 45016 Fresno, CA 93718.
